𝗨𝗻𝗱𝗲𝗿𝘀𝘁𝗮𝗻𝗱𝗶𝗻𝗴 𝗛𝘆𝗽𝗲𝗿𝘁𝗲𝗻𝘀𝗶𝘃𝗲 𝗖𝗿𝗶𝘀𝗶𝘀: Many high-net-worth principals carry silent hypertension — undiagnosed, undisclosed, and undertreated. For EP professionals, this isn’t a background detail. It’s a ticking operational variable.
𝗪𝗵𝗮𝘁 𝗖𝗮𝘂𝘀𝗲𝘀 𝗶𝘁?
Dangerously elevated blood pressure (≥180/120 mmHg), often triggered by:
• Missed medications or undisclosed noncompliance
• Extreme stress — think high-stakes negotiations, public events, or threat exposures
• Stimulant use (caffeine, decongestants, illicit substances)
• Sleep deprivation during demanding travel schedules
• Underlying kidney disease or hormonal disorders
𝗦𝗶𝗴𝗻𝘀 𝗮𝗻𝗱 𝗦𝘆𝗺𝗽𝘁𝗼𝗺𝘀
Be vigilant — this can look like stress or fatigue:
• Severe headache, often described as “the worst of my life”
• Blurred or double vision
• Chest pain or pressure
• Shortness of breath
• Nausea, confusion, or altered mental status
• Nosebleeds (less common but notable)
𝗡𝗼𝘁𝗲: A hypertensive emergency means end-organ damage is occurring: brain, heart, and kidneys are at risk. Minutes matter.
𝗥𝗶𝘀𝗸 𝗙𝗮𝗰𝘁𝗼𝗿𝘀
• Controllable: Medication noncompliance, poor diet, high stress, stimulant use
• Uncontrollable: Age, genetics, pre-existing kidney/cardiac disease
𝗧𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁
• Remove the principal from the stressor environment immediately
• Call 911 — this is a medical emergency, not a “wait and see”
• Keep them calm, seated, and still — exertion worsens outcomes
• Do NOT give them medications unless directed by medical professionals
• Know their baseline: Does the principal take blood pressure medications? What is their normal BP?
• Document time of symptom onset for ER handoff
𝗧𝗮𝗸𝗲 𝗔𝗰𝘁𝗶𝗼𝗻 𝗧𝗼𝗱𝗮𝘆
• Know the Signs: Severe headache + vision changes + chest pain = call 911 now
• Build the Medical Brief: Confirm whether your principal has a hypertension history
• Operational Prep: Carry a portable BP cuff. Baseline vitals are intelligence.
